Nurse / Qualified Professional
Bring your nursing experience to home-based care. RNs and LPNs at HAAS provide skilled-care oversight, support care planning, and visit clients to ensure care quality. You'll work with caregivers, families, and case managers to help clients live independently and safely.
What you’ll do
The day-to-day.
- Conduct assessments and contribute to care plan development.
- Provide skilled-nursing tasks as authorized in client care plans.
- Supervise and train PCAs and 245D direct support staff on health-related tasks.
- Coordinate with primary-care providers, case managers, and families.
- Document visits and clinical observations.
- Respond to changes in client condition and adjust care plans accordingly.
What we’re looking for
Qualifications.
- Active RN or LPN license in good standing in Minnesota.
- Prior nursing experience preferred — home health, geriatrics, disability services, or community health especially.
- Willingness to travel to client homes across the Twin Cities metro.
- Comfort with electronic documentation and care planning systems.
- Excellent communication and supervisory skills.
